How do I schedule an appointment?
After receiving a PT order from your MIT provider, call or visit the office to schedule an appointment. Have your order and insurance card available when you make your first appointment. This will allow us to verify your insurance benefits and provide you with an estimate of coverage.
What do I need to know before my first appointment?
Bring an order for physical therapy (if needed) from your referring physician or nurse practitioner and your insurance information (we will make a copy of your card) to your first appointment. Print out the patient intake form [PDF] in advance, fill it out, and bring it with you. If you are receiving physical therapy for an automobile accident or worker’s compensation claim, bring relevant information related to that claim — the name(s) and phone number(s) of your case manager or insurance adjuster and your claim number.
Arrive 15 minutes early to complete any remaining paperwork, so we have plenty of time to complete your evaluation. Wear comfortable clothing that allows easy access to the part of your body we will be evaluating.

Will insurance cover my treatment?
Yes, in most cases. We are in-network providers for all of the MIT-sponsored Blue Cross Blue Shield of Massachusetts plans. Our office can contact your insurance company and determine your coverage. How many sessions will I have?
Each patient's diagnosis is different, so your physical therapist will work with your referring provider to develop a plan of care that is right for you. The evaluation at your first visit will determine your plan of care and number of visits. Your physical therapist will explain the plan at that time.
